What happened in P&A:
The DOPs was in the best intention written very detailed, with exact description of many small tasks in the work process.

What Was Planned

The first DOPs for running the landing string and thereafter running slickline and wireline was review with operational staff from the service companies on the rig.

What Was Learned

The service hands on the rig explained that they could not guarantee that all the operational steps would actually be done in the same order, and some steps were also wrong. They recommended that the DOPs were written in a more general form, with less detail.

What Actions Are Required

Use terms like “rig up wireline and arm the cutting tool according to Archer procedure and offshore supervision” As long we have experienced supervisors from the vendors present on the rig, we can make life easier for all by simplifying the DOPs and trust and empower people.

What Was Done

The DOPs were re-written and corrected after the review meeting offshore
